Combine cream cheese, chicken broth, chicken breasts, and mushrooms in a crockpot. Cover and cook on low for 4-5 hours.
When chicken is cooked through, stir well to make sure broth/cream cheese/mushrooms are mixed.
Place a slice of swiss cheese on each chicken breast and sprinkle with crumbled bacon. Turn crockpot up to high, cover, and cook an additional 10-15 minutes until the cheese is melted.
